You will need to ensure you have this form filled in prior to your visit to the skydive centre. Please download appropriate form and print it out.
If you are under 40 years of age you will need to 'self certify' by filling in the appropriate section of the medical form.
If you are over 40 years of age you will be required to get your doctor to sign the medical form to say you are fit to parachute.
